Clinical Overview of Bone Cyst Curettage: Procedure, Indications, and Recovery

Bone cyst curettage is a specialized orthopedic surgery performed to scrape out a benign (non-cancerous), fluid-filled cavity or lesion inside a bone. While these cysts are not malignant, they expand over time, thinning the surrounding dr abhishek kumar ortho healthy bone walls and significantly reducing the bone’s structural integrity. This leaves the patient vulnerable to minor impacts causing severe fractures. The primary objectives of the surgery are to thoroughly eradicate the cystic tissue, stimulate new bone growth, and prevent localized skeletal collapse.

The Surgical Procedure

Bone cyst curettage is typically performed under general or regional anesthesia and follows a precise structural sequence:

  1. Access and Windowing: The surgeon makes an incision over the affected bone area and cuts a small “window” through the hard outer shell of the bone to directly access the internal cavity.
  2. Curettage: Using specialized, spoon-shaped surgical instruments called curettes, the surgeon meticulously scrapes out all the abnormal lining and fluid inside the cavity.
  3. Adjuvant Therapy (Optional): To ensure no microscopic cyst cells remain, the surgeon may chemically treat the empty cavity walls with an adjuvant material, such as liquid nitrogen, alcohol, or phenol.
  4. Bone Grafting: The remaining structural void is packed tightly with a filling material to jumpstart bone regeneration. This can be an autograft (bone harvested from the patient’s hip), an allograft (donor bone tissue), or synthetic bone substitute materials.

Primary Clinical Indications

This surgical intervention is indicated for specific non-cancerous bone lesions:

  • Unicameral Bone Cysts (UBCs): Simple, fluid-filled single cavities commonly found in the growing long bones of children and teenagers.
  • Aneurysmal Bone Cysts (ABCs): More aggressive, blood-filled vascular lesions that tend to expand rapidly, causing significant bone destruction and localized pain.
  • High Fracture Risk: Any benign cystic lesion that has weakened the bone wall to the point where a pathological fracture is imminent or has already occurred.

What to Expect During Recovery

The post-operative timeline focuses heavily on protecting the bone graft while the body replaces it with dense, mature bone.

Activity Modification: Weight-bearing restrictions or protective splinting/bracing are mandatory for the first 4 to 8 weeks if the operation was performed on a weight-bearing bone, such as the femur or tibia.

Phased Rehabilitation: Gentle physical therapy begins early to preserve joint mobility and prevent nearby muscle wasting, gradually building up to resistive strengthening as the bone heals.

Long-Term Monitoring: Periodic follow-up X-rays or MRI scans are necessary over 6 to 12 months to ensure the bone graft is successfully incorporating and to monitor for potential cyst recurrence, which can happen before the bone fully matures.
